29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: (a) BOH, Inc. at 13765 55th Street NW, Williston, ND 58801: On or before November 1, 2011, the employer did not ensure that three employees were protected from falling when sheathing the roof deck. Condition exposed employees to fall hazards of up to 16 feet from ground-to-eave.

29 CFR 1926.102(a)(3)(ii): Employees whose vision requires the use of corrective lenses in spectacles, when required by this regulation to wear eye protection, shall be protected by goggles that can be worn over corrective spectacles without disturbing the adjustment of the spectacles: (a) BOH, Inc. at 13765 55th Street NW, Williston, ND 58801: On or before November 1, 2011, the employer did not ensure that one employee who was working in the vicinity of a pneumatic nail gun, and who was wearing prescriptive eye glasses, was protected from projectiles to the eyes through the use of goggles that fit over the prescription glasses. Condition exposed this employee to the hazard of projectiles to the eyes.
