29 CFR 1926.416(a)(1):  Employees were permitted to work in close proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or by effectively guarding the circuits by insulation or other means:    (a)  Triangle Electric, Inc. at 13593 56th Street NW, Williston, ND 58801:  On or about November 2, 2011, the employer permitted an employee to remove the cover panel and some circuit breakers of a 120/240-volt electrical panel without de-energizing and grounding the circuits and without effectively guarding the circuits.  Condition exposed this employee to electrical shock, arc flash, and arc blast hazards.
