OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that the employees installing the insulated metal wall panel were not protected from the wall panels falling in the event of  vacuum lift failure.     A.  Sawyer Structural Steel Inc. worksite located at 2125 Wood Bridge Blvd., Bowling Green, OH:  On or about October 7, 2011, the employer did not ensure the secondary fall safety straps were used while using a vacuum lift to hoist the long insulated wall panels near column line Z exposing employees to falling panels in the event of a vacuum failure.     B.  Sawyer Structural Steel Inc. worksite located at 2125 Wood Bridge Blvd., Bowling Green, OH:  On or about December 1, 2011, the employer did not ensure the secondary fall safety straps were used while using a vacuum lift to hoist the short insulated wall panels on the south side of the structure near the parking lot exposing employees to falling panels in the event of a vacuum failure.     The AVD is amended to add instance C:  Sawyer Structural Steel Inc., worksite located at 2125 Wood Bridge Blvd., Bowling Green, OH:  On or about October 7, 2011, the employer did not assure Employees were protected from falling tools.  The bar clamp was Not secured to the aerial lift and/or the superstructure      Abatement Note: Among other methods, one feasible and acceptable abatement method to correct this hazard would be to follow the manufacturer's recommendations outlined in the Cool Boy Instruction Manual requiring the use of a secondary fall safety strap.

OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that the employees were exposed to falling tools. An employee was struck by a falling bar clamp resulting in a massiv concussion.    A.  Sawyer Structural Steel Inc. worksite located at 2125 Wood Bridge Blvd., Bowling Green, OH:  On or about October 7, 2011, the employer did not ensure employees were protected from falling tools. An employee was struck by a bar clamp that fell approximately 35.00 feet which was used to keep the panel seams tight. The bar clamp was not secured to the aerial lift and/or the superstructure.     Abatement Note: Among other methods, one feasible and acceptable abatement method to correct this hazard would be to follow the recommendations implemented by the employer on October 9, 2011 indicating all bar clamps and other elevated tools and similar equipment will be tied-off with its own lanyard to prevent falling to the ground if dropped or if becoming unsecure.
