29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees. a) An employee entered the envelope of movement of the Fanuc Robot #1 in Building A, without a remote control teach pendant, in order to access a control panel, but failed to lock out the power source of the robot. b) Employees were routinely entering the envelope of movement of robots in Building A in order to perform end of week cleaning, but failed to lockout the power source of the robots.
