Crossbraces shall not be used as a means of access. a. On or about December 1, 2011, at the above addressed jobsite, employees conducting roofing operations from a ladder jack scaffold, twenty four (24) feet above the ground, were not provided with an access ladder, thereby exposing the employees to falls.
