29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms, and service rooms were not kept clean and orderly and in a sanitary condition.    a) Bagging area: On or about December 1, 2011 and at times prior thereto, employees were exposed to a fire hazard where a build up of wood product had accumulated below the bagging shed.    b) The plant: On or about December 1, 2011 and at times prior thereto, employees were exposed to a slip and trip hazard while using a set of stairs which had a large build up of wood product on each step.    c) On or about December 1, 2011 and at times prior thereto, employees were exposed to hazards such as but not limited to fire where a build up of wood product existed in areas such as but not limited to the bagging area, Plant area and the grinder area.

29 CFR 1910.212(a)(1):     Types of guarding. One or more methods of machine guarding was not provided to protect the operatorand other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc.    a) Bagging area: On or about December 1, 2011 and at times prior thereto, employees were exposed to the unguarded pinch points on the unguarded chain drag.    b) Bagging area: On or about December 1, 2011 and at times prior thereto, employees were exposed to the unguarded pinch point on one of the bark feed belts which has an old motor being used as a counter weight.    c)  The Plant: On or about December 1, 2011 and at times prior thereto, employees were exposed to the unguarded pinch point on the unguarded feed belts.    d) The Grinder: On or about December 1, 2011 and at times prior thereto, employees were exposed to the unguarded pinch point on the unguarded chain drag at the tail pulley and at the supports.    e) Shop Area: On or about December 1, 2011 and at times prior thereto, employees were exposed to the unguarded rotating chuck on the Enco drill press.

29 CFR 1910.219(f)(3): Sprockets and chains. All sprocket wheels and chains were not enclosed unless they are more than seven (7) feet above the floor or platform. Where the drive extends over other machine or working areas, protection against falling shall be provided. This subparagraph does not apply to manually operated sprockets.    a) Bagging area: On or about December 1, 2011 and at times prior thereto, employees were exposed to an amputation hazard while working on and near the unguarded chain and sprockets located out side the access door to the bagging room.    b) Material sizing equipment: On or about December 1, 2011 and at times prior thereto, employees were exposed to an amputation hazard while working on or cleaning up near the unguarded chain and sprocket approximately 6 feet 2 inches above the ground.    c) The plant/separator: On or about December 1, 2012 and at times prior thereto, employees were exposed to an amputation hazard while walking and working in the area of the drive chain on the large separator.

29 CFR 1910.305(b)(2)(i): All pull boxes, junction boxes, and fittings were not provided with covers identified for the purpose. If metal covers are used, they shall be grounded. In completed installations, each outlet box did not have a cover, faceplate, or fixture canopy. Covers of outlet boxes having holes through which flexible cord pendants pass shall be provided with bushings designed for the purpose or shall have smooth, well-rounded surfaces on which the cords may bear.    a) Grinder Building: On or about December 1, 2011 and at times prior thereto, employees were exposed to a shock hazard while walking and working in the area of the building referred to as the grinder from an out door junction box with no cover caring 480 three phase power.      b) Conveyor to the grinder: On or about December 1, 2011 and at times prior thereto, employees were exposed to electrical shock while cleaning and maintaining in the elevated out door area of the grinder where covers are not in place on the 480 three phase system.    c) Conveyor to the grinder: On or about December 1, 2011 and at times prior thereto, employees were exposed to a shock hazard while plugging in lamps, while working on and with a 120 volt outlet missing its protective cover.    d) Maintenance shop: On or about December 1, 2012 and at times prior thereto, employees were exposed to a shock hazard from the energized green outlet located on the outside of the maintenance shop near the door used to enter the shop.
