29 CFR 1926.501(b)(1): Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:     a) Filters building A & B: On November 30, 2011 and times prior thereto employees worked on a one in twelve pitched metal roof installing multiple solar tubes without a fall protection system in place.         Note) Abatement certification is NOT required for this item.

29 CFR 1926.503(a)(2)(iii): The employer did not assure that each employee had been trained as necessary, by a competent person qualified in the use and operation of guardrail systems, personal fall arrest systems, safety net systems, warning line systems, safety monitoring systems, controlled access zones, and other protection to be used:     a) Jobsite: On November 30, 2011 and times prior thereto, employees and the responsible person on site did not acknowledge the hazard of working on a low sloped roof with unprotected sides and edges, and take the necessary actions to eliminate or minimize the hazard.       Note) Abatement certification IS required for this item.
