OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by, crushing, and amputation hazards from improperly erected rig superstructure and improperly assembled tongs.      a) On or about October 29, 2011 and at times prior, Cyclone Drilling Inc did not ensure that employees were protected from struck-by, crushing, and amputation hazards.      1.  Employees were exposed to structural collapse due to several connecting pins not being installed during the erection of the rig substructure.    2.  Employees were exposed to mechanical faillure while operating the tongs on the rig floor.  The tongs were assembled using bolts instead of proper sized pins and keepers.

Portable fire extinguishers were found, which had broken and bent pins, that could not be pulled to activate the extinguisher.

29 CFR 1910.219(c)(2)(i):  All exposed part(s) of horizontal shafting seven (7) feet or less from floor or working platform were not protected by stationary casing(s) enclosing shafting completely or by trough(s) enclosing sides and top or sides and bottom of shafting:    a) On or about October 29, 2011, and at times prior, Cyclone Drilling Inc. did not ensure that employees were protected from moving mechanical parts.  The pony rods in the mud pumps area were inadequately guarded.  The top spread metal had been cut open to allow for easy access during maintenance operations.

Cyclone Drilling, Inc., was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard 1910.151(c) which was contained in OSHA inspection 314658196 citation number 1 item 2 issued on 11/09/2010, and became a final order on 12/17/2010 with respect to a workplace located at Federal RGU #543-24-198, Rig #29, Meeker, CO 81641.
