29 CFR 1910.132(a): Protective equipment was not maintained in a sanitary and reliable condition: Employees working in the trailer wash bay to clean tanker trucks were exposed to fall hazards from an approximate height of 11.5 ft to 13.5 feet and the employer did not provide reliable fall protection and require all employees exposed to a fall use the protective equipment.

1910.253(b)(2)(ii) Inside of buildings, cylinders should be stored in a well-protected, well-ventilated, dry location, at least 20 (6.1 m) feet from highly combustible materials such as oil or excelsior. Cylinders were not stored in definitely assigned places away from elevators, stairs, or gangways. Assigned storage spaces were not located where cylinders will not be knocked over or damaged by passing or falling objects, or subject to tampering by unauthorized persons. Cylinders were not kept in unventilated enclosures such as lockers and cupboards. Employees working in the mechanic bay were exposed to burns due to cylinders not secured properly Chain to secure fuel cylinders from being knocked over was secured towards the bottom of the cylinder.

29 CFR 1910.253(b)(4)(iii) Oxygen cylinders in storage were not separated from fuel-gas cylinders or combustible materials (especially oil or grease), a minimum distance of 20 feet (6.1 m) or by a noncombustible barrier at least 5 feet (1.5 m) high having a fire-resistance rating of at least 1/2 hour. Employees working in the mechanic bay were exposed to burns due to fuel cylinders stored next to oxygen cylinders without having a minimum distance of 20 feet (6.1 m) or a noncombustible barrier at least 5 feet (1.5 m) high having a fire-resistance rating of at least 1/2 hour.
