29 CFR 1910.305(g)(2)(iii):     Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws:    On or about 12/06/11 during lighting maintenance activities, the employer did not ensure employees examined a mobile scissor lift.  The electrical plug on the back of the unit was missing electrical insulation between the cord and the plug.
