29 CFR 1910.29(a)(3)(viii): Guardrails not less than 2 x 4 inches or the equivalent and not less than 36 inches or more than 42 inches high, with a mid-rail, when required, of 1 x 4-inch lumber or equivalent, and toeboards, were not installed at all open sides on all scaffolds more than 10 feet above the ground or floor: On or about 12/06/11 during lighting maintenance activities, the employer did not ensure employees were protected by a guardrail system when using a mobile scissor lift. The employee stood on the mid rails to conduct work activities. He was exposed to a fall in excess of 25 feet.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws: On or about 12/06/11 during lighting maintenance activities, the employer did not ensure employees examined a mobile scissor lift. The electrical plug on the back of the unit was missing electrical insulation between the cord and the plug.
