On or about December 6, 2011, at a jobsite located at 234 East Marquette Road, Chicago, IL 60637  Employees performing power washing of walls from a ladder jack scaffold approximately eleven (11) feet above ground level were not protected by the use of personal fall arrest systems, thereby exposing the employees to fall hazards.
