CFR 1926.501(b) (13):  Each employee engaged residential construction activities 6 feet  (1.8 m) or more above lower levels was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems;    (a)	M & C Construction LLP, at 5192 E. 119th Way, Thornton, CO:  On and before November 30, 2011, five employees working on 5:12 pitch, two story, single family home roof were not protected from falling by a system as described above.  Specifically, three employees were leaning near the edge of the roof to perform roofing work and two employees were on the first floor section of roof removing old shingles.  This condition exposed employees to fall hazards of over 10 feet on the lower section of the roof and approximately 25 feet on the second story.

CFR 1926.503(a)(1):  The employer failed to provide a training program for each employee who might be exposed to fall hazards. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards.    (a)	M & C Construction, LLP at 5192 E. 119th Way, Thornton, CO 80233:  On and before November 30, 2011, six employees were working on a two story single family home with fall heights up to 25 feet without adequate fall protective measures in place.  The employer did not provide each employee with training which would enalbe them to recognize fall hazards and how to minimize these hazards.  This condition exposed employees to fall hazards up to 25 feet.

CFR 1926.1053(b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails failed to extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access; or, when such an extension was not possible because of the ladder's length, the ladder was not secured at its top to a rigid support that did not deflect, and a grasping device, such as a grab rail was not provided to assist employees in mounting and dismounting the ladder    (a)	M & C Construction LLP, at 5192 E. 119th Way, Thornton, CO 80233:  On and before November 30, 2011, six employees working on a two story single family home re-roofing project utilized three ladders for accessing an upper landing without ensuring that the ladder's side rails extended at least three feet above the upper landing.  This condition exposed employees to fall hazards of up to 25 feet.
