29 CFR 1910.219(f)(3):     Sprockets and chains. All sprocket wheels and chains shall be enclosed unless they are more than seven (7) feet above the floor or platform. Where the drive extends over other machine or working areas, protection against falling shall be provided.This subparagraph does not apply to manually operated sprockets.    a) At the Royal Pad Products facility located in Lancaster, Ohio the employer had an unguarded chain and sprocket that was located approximately 2-3 feet from the floor on the brick sheeter machine within 2-3 feet of the operators station.    b) At the Royal Pad Products facility located in Lancaster, Ohio the employer had several chain and sprockets that were located on the # 1 Sheeter by the expandable pulley section, that were not adequately guarded.  The employer had used fencing material to block access to the gears, but the fencing guard just rested on the ground and was not securely mounted to the floor or machine as required in 1910.219(m)(1)(1).

29 CFR 1910.219(e)(3)(i):     Vertical and inclined belts shall be enclosed by a guard conforming to standards in paragraphs(m) and (o) of this section.    a) At the Royal Pad Products facility located in Lancaster, Ohio the employer had an unguarded belt that was on the #2 cutter.  The #2 cutter had an approximately 2.25 inch flat belt that ran between the Baldor industrial motor (model # M2506T) and the flywheel that operated the shear of the #2 cutter.    b) At the Royal Pad Products facility located in Lancaster, Ohio the employer had an unguarded belt that was on the #2 sheeter.  At the expandable pulley section of the Clark Aiken #2 sheeter the guard had been removed and was not replaced.

29 CFR 1910.219(b)(1):     Flywheels. Flywheels located so that any part is seven (7) feet or less above floor or platform shall be guarded in accordance with the requirements of this subparagraph outlined in 1910.219(b)(1)(i) through 1910.219(b)(1)(vi):     a) At the Royal Pad Products facility located in Lancaster, Ohio the employer had an unguarded flywheel on the #2 cutter.  The expanded metal guard had been removed exposing the flywheel to pedestrian traffic.

29 CFR 1910.24(h):     "Railings and handrails." Standard railings shall be provided on the open sides of all exposedstairways and stair platforms. Handrails shall be provided on at least one side of closed stairways preferably on the right side descending. Stairrailings and handrails shall be installed in accordance with the provisions of 1910.23.    a) At the Royal Pad Products facility located in Lancaster, Ohio the employer had fixed industrial stairs that went from the main floor up to the catwalk that led to the upstairs roll saw that did not have an adequate railing at the upper treads of the stairway.  At top of the stairs the top rail of the stairway railing was approximately 26 inches from the top of the tread in line with the riser.

Hard-service cord and junior hard-service cord No. 14 and larger may be repaired if spliced so that the splice retains the insulation, outer sheath properties, and usage characteristics of the cord being spliced.    a) At the Royal Pad Products facility located in Lancaster, Ohio the employer had multiple surge protectors daisy-chained together in the office.

29 CFR 1910.305(g)(1)(iv)(A): Unless specifically permitted otherwise in paragraph (g)(1)(ii) of this section, flexible cords and cables may not be used:    As a substitute for the fixed wiring of a structure;    a) At the Royal Pad Products facility located in Lancaster, Ohio the employer had used extension cords as a substitute to fixed wiring to provide power to heat tape that was wrapped around the water separators that were installed on the compressed air system throughout the plant.
