29 CFR 1910.1200(e)(1): Employers shall develop, implement, and maintain at each workplace, a written hazard communication program which at least describes how the criteria specified in paragraphs (f), (g), and (h) of this section for labels and other forms of warning, material safety data sheets, and employee information and training will be met, and which also includes the following: a. On or about December 6, 2011, employees were required to work with chemicals including but not limited to, triethanolamine and hydrotreated naphthenic oil. The employer had not developed and implemented a written hazard communication program including, but not limited to proper labeling, material safety data sheets and training procedures.

29 CFR 1910.1025(d)(2): Initial determination. Each employer who has a workplace or work operation covered by this standard shall determine if any employee may be exposed to lead at or above the action level. a. On or about December 6, 2011, employees were required to perform machining operations on brass, a material which contains lead. The employer failed to perform an initial determination to evaluate employee exposure to lead when conducting these operations.

29 CFR 1910.1026(d)(1): General. Each employer who has a workplace or work operation covered by this section shall determine the 8-hour TWA exposure for each employee exposed to chromium (VI). This determination shall be made in accordance with either paragraph (d)(2) or paragraph (d)(3) of this section. a. On or about December 6, 2011, employees were required to weld on stainless steel which can produce exposure to chromium (VI). The employer failed to determine the employee 8 hour TWA exposure to chromium (VI).
