29 CFR 1926.1053(b)(1):     Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder���s length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigied support that will not deflect, and/or a grasping device was not provided to assist emmployees in mounting and dismounting the ladder:    On or about December 7, 2011, during construction activities including but not limited to carpentry/framing activities on a steep roof, the employer did not ensure employees using a ladder to access the roof were protected from fall hazards approximately 9 feet, by ensuring the ladder extended at least three feet above the roof surface or a grab rail was provided for mounting/dismounting the ladder.

29 CFR 1926.102(a)(1):     Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:    On or about December 7, 2011, during construction activities including but not limited to carpentry/framing activities on a steep roof, the employer did not ensure employees were protected from eye hazards by ensuring eye protection was available and worn during nailing and framing activities.     A & D CONSTRUCTION  WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD, 29 CFR 1926.102 (a)(1), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 314605502, CITATION NUMBER 01, ITEM NUMBER 001, AND WAS AFFIRMED AS A FINAL ORDER ON JULY 20, 2010, WITH RESPECT TO A WORKPLACE LOCATED AT 359 LAKE PARK ROAD, BAY VILLAGE, OH 44140.

29 CFR 1926.501(b)(13):     Employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph (b) of this section provides for an alternative fall protection measure. Note:  If using these systems was infeasible or created a greater hazard, the employer should develop and implement a fall protection plan meeting the requirements of paragraph (k) of 1926.502:    On or about December 7, 2011, during construction activities including but not limited to carpentry/framing activities on a steep roof, the employer did not ensure employees were protected from fall hazards approximately 24 feet on main house and 13 feet on garage, by ensuring a fall protection system system/device was being used.     A & D CONSTRUCTION  WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD, 29 CFR 1926.501 (b)(13), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 314605502, CITATION NUMBER 01, ITEM NUMBER 002, AND WAS AFFIRMED AS A FINAL ORDER ON JULY 20, 2010, WITH RESPECT TO A WORKPLACE LOCATED AT 359 LAKE PARK ROAD, BAY VILLAGE, OH 44140.
