29 CFR 1910.303(b)(2):     Installation and use. Listed or labeled equipment was not installed and used in accordance with any instructions included in the listing or labeling.    Throughout Facility.  Electrical pendant boxes with multiple outlets (110v) were used on the ends of flexible cables and hung above work stations where employees plugged in portable devices such as fans and pumps.  The boxes had pre-punched knockouts, therefore exposing employees to electrical shock hazards when they used one hand to stabilize the box while plugging a device in with the other hand.    Abatement certification is required for this item in accordance with 29 CFR 1903.19(c).
