29 CFR 1910.67(c)(2)(v): A body belt was not worn and a lanyard attached to the boom or basket when working from an aerial lift: a) Located in the parking lot of a retail mall, employees were observed working on an elevated lighting system light pole performing this operation from within the basket of a Bil-Jax 4527A extensible and articulating boom aerial lift without lanyards attached to the boom or basket and were exposed to a fall potential in excess of 15'. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

29 CFR 1910.333(a): Safety-related work practices were not employed to prevent electric shock or other injuries resulting from either direct or indirect electrical contacts, when work was performed near or on equipment or circuits which were or could be energized: a) On or about December 7, 2011 employees observed performing light pole 204v-208v lighting system removal and replacement associated operations did not employ electrical safety related work pratices to prevent electrocution, electrical shock or other injuries from direct or indirect contact when work was performed on equipment or circuits which could become energized. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.
