29 CFR 1910.147(e)(3): The employer's energy control program did not contain specific procedures and training for when the authorized employee who applied the lockout or tagout device was not available to remove it, and the device was removed under the direction of the employer:  (a)  Throughout the facility; An additional key for each lockout device was unsecured in the maintenance area. In addition; some lockout lock sets located at the lockout stations had two keys available and the employer did not develop, document, and train employees on specific procedures for lock removal by personnel other than the individual whom applied the device.
