29 CFR 1910.36(b)(1): At least two exit routes were not available in a workplace to permit prompt evacuation of employees and other building occupants during an emergency: a. Facility - On or about September 22, 2011, employees were required to work in the facility with one only exit located on the east side of the building. The employees did not have at least two exit routes available and thereby were exposed to the hazards associated with fires and smoke inhalation.

29 CFR 1910.303(g)(2)(i): Except as elsewhere required or permitted by this standard, live parts of electric equipment operating at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the following means: a. Air compressor area - On or about September 22, 2011, employees were required to work with and operate the Powerex air compressor system near a disconnect box with exposed wires (220 Volts). Employees were thereby exposed to the electrical hazards associated with live parts. b. Barrel washer area - On or about September 22, 2011, employees were required to work with and operate the barrel washer system near an open junction box with exposed wires (120 Volts). Employees were thereby exposed to the electrical hazards associated with live parts. c. Chiming area - On or about September 22, 2011, employees were required to open a 220 Volt relay control panel door to operate the barrel conveyor system. Employees were thereby exposed to the electrical hazards associated with live parts.
