29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards: a. Establishment, Pembroke, NH - Employees who enter the roof area on a weekly basis for inspection and maintenance of the ammonia refrigeration piping system were exposed to a fall hazard without standard railings or equivalent protection. Note: To abate this hazard, the employer may install a permanent fall protection system (e.g., guardrail system), attachment (tie-off) point so that fall protection is readily available when needed, or a designated area (delineated with a line and stanchions. With a guardrail system, toeboards are only required when persons can pass, there is moving equipment, or there is equipment with which falling materials could create a hazard. For the designated area abatement option consult OSHA Proposed Rule 1910.28 and 1910.29 dated 5/24/2010.

Control Room for ammonia refrigeration system - The squeeze bottle eye wash station located in this room was not suitable for quick drenching or flushing of the eyes and body in the event of an emergency.
