29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section: a) On or about October 30, 2011 and at times prior, Central NDT Inc as the controlling contractor, did not ensure that workers were protected from excavation hazards. Company representatives on site did watch crews of the subcontractor install dunnage and lay out pipe for the welder making the connections.
