29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:    a)  On or about November 1, 2011, employees were not protected from falls from the basket of an aerial lift in that, fall restraint or arrest systems were not used to prevent employees from falling from the basket.

29 CFR 1926.760(b)(3): Each connector working at heights over 15 and up to 30 feet above a lower level, was not provided with a personal fall arrest system, positioning device system or fall restraint system and wear the equipment necessary to be able to be tied off; or was not provided with other means of protection from fall hazards in accordance with paragraph (a)(1) of this section:    a)  On or about November 1, 2011 and at times prior, Sion Steel Erectors did not ensure that employees were protected from fall hazards.  Employees constructing a prefabricated structure were working at heights above 15 feet without wearing any type of fall protection equipment to afford them the ability to tie off or were not provided with other means of fall protection.
