Crews of nonelectrical workers did work in the electrical rooms where the panels were live and had inadequate or no covers.

29 CFR 1926.405(b)(1): Conductors entering boxes, cabinets, and fittings were not protected from abrasion: a) On or about November 2, 2011 and at times prior, Sancho Constructors LLC as the controlling contractor did not ensure that workers were protected from electrical hazards. Crews of nonelectrical workers were working in the electrical rooms where improper connections had been made. The conductors feeding the temporary panels were not protected from abrasion, as they were strung over the edge of the panel boxes.
