The platform did not have fall protection on the entrance (chains), and the mezzanine did not have guardrails.      Among other methods, one feasible and acceptable method to correct this hazardous condition is to ensure that the fall hazards are corrected according to ANSI Z359.2-2007, Minimum Requirements for a Comprehensive Managed Fall Protection Program.
