OSH ACT of 1970 Section (5)(a)(1):     The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees  in that employees were exposed to slipping on and falling from a free-turning roller conveyor.    (a)  On the Viking automatic pallet nailer, employee(s) were permitted stand on the free-turning roller conveyors to center pallets in the stacker and/or make adjustments on the stacker's lifting arms. Employee(s) were exposed to fall hazards.

The piece of equipment was solely placed in manual mode. Employees were exposed to struck-by and/or entanglement hazards.

29 CFR 1910.213(b)(3):     Provisions were not made to prevent woodworking machine(s) from automatically restarting upon restoration of power after power failure:      (a)  In the pallet assembly area, the JET vertical band saw blade would automatically restore power and cycle after a power failure. There were no controls to reset the machine prior to starting back up.
