29 CFR 1910.1025(e)(1): Feasible engineering and work practice controls (including administrative controls) were not implemented to reduce employee exposure to or below the permissible exposure limit:    On or about January 12, 2012, at the Mirror Department, an employee required to perform batch operations was exposed to airborne lead dust at an 8-hr time-weighted average (TWA) level of 220 micrograms per cubic meter of air, approximately 4.4 times the permissible exposure limit of 50 micrograms (8-hr TWA).  Feasible engineering and work practice controls (including administrative controls) were not implemented to reduce employee exposure to or below the permissible exposure limit or lowest feasible level.    General methods of control that may be applicable to the process include, but are not limited to, the following:    1. Implement local exhaust ventilation (LEV) engineering control improvements as recommended in the Liberty Mutual lead survey report dated 03/20/2008.    2. Evaluate the efficiency (i.e. capture velocity, duct velocity, static pressure) of the existing LEV system servicing the mixing vessels to ensure performance in accordance with the American Conference of Governmental Industrial Hygienists (ACGIH) Ventilation Manual.    Abatement normally will be multistep as follows:    STEP 1: Submit to the Area Director, a written detailed plan of abatement outlining a schedule for the implementation of engineering and/or administrative measures to control employee exposure to lead.   This plan shall include, at a minimum, target dates for the following actions which must be consistent with the abatement dates required by this citation:    1. Evaluation of engineering/administrative control options;  2. Selection of optimum control methods and completion design;  3. Procurement, installation, and operation of selected control measures;  4. Testing and acceptable or modification/redesign of controls.    All proposed control measures shall be approved for each particular use by a competent Industrial Hygienist or other technically qualified person.    Abatement Period: 60 Days                        Date by Which Violation Must be Abated                  05/11/2012    STEP 2: Abatement shall have been completed by the implementation of feasible engineering and/or administrative controls upon verification of their effectiveness in achieving compliance.    Abatement period: 90 days
