29 CFR 1926.403(i)(2)(i): Live parts of electric equipment operating at 50 volts or more were not guarded against accidental contact by cabinets or other forms of enclosures, nor by any of the means listed in subparagraphs (A) through (D) of this paragraph. a) On or about November 2, 2011 and at times prior, Electrical Systems Inc. as the creating/exposing contractor did not ensure that employees/workers were protected from electrical hazards. Crews of subcontractors and electricians did work in the electrical rooms where the panels were live and had inadequate or no covers.

29 CFR 1926.405(b)(1): Conductors entering boxes, cabinets, and fittings were not protected from abrasion: a) On or about November 2, 2011 and at times prior, Electrical Systems Inc. as the creating/exposing contractor did not ensure that employees/workers were protected from electrical hazards. Crews of different skills including the electricians who did the work were exposed to improper conductor installation. The conductors feeding the temporary panels were not protected from abrasion, as they were strung over the edge of the panel boxes.
