29 CFR 1915.74(a)(2): The employer permitted employees to board or leave a vessel by using a gangway, that did not have on each side a railing with a minimum height of approximately 33 inches, measured perpendicularly from rail to the walking surface. The floating dock, on both ends of the barge, lacked fall protection.

29 CFR 1915.75(e): When employees working on the floor of a floating dry dock were exposed to the hazard of falling into the water, the end of the dry dock was not equipped with portable stanchions and 4inch railings with a midrail. The North and South end of the floating dry dock were not protected.
