OSH ACT of 1970 Section (5)(a)(1):  The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm including burns, shocks, and electrocution hazards to employees:    a)  On or about October 6, 2011 and at times prior, Rakhra Mushroom Farm did not ensure that employees were protected from electrical hazards.  Employees involved in cleaning, slicing, packaging, and area clean up were exposed to:    1.  Electrical cords which had strain relief problems    2.  Electrical cords which were missing ground prongs    3.  Electrical boxes which had missing knockouts    4.  Electrical components used in wet and damp locations which were not rated for such operations.    5.  Damaged electrical boxes and conduit which had deteriated to the point of exposing the conductors.  This item should have been addressed during the required examination of the electrical systems.    Among other methods, one feasible and acceptable abatement method to correct this hazard is:  Develop and implement a program which would call for inspections of the electrical systems, repair immediately of all hazards noted, and training of the crew in the recognition and avoidance of electrical hazards.
