29 CFR 1910.134(f)(2): The employer did not ensure that employees using a tight-fitting facepiece respirator were fit tested prior to initial use of the respirator, whenever a different respirator facepiece (size, style, model or make) is used, or at least annually thereafter: (a) Cornerstone Granite Inc. at 940 S. Jason St. Unit 9, Denver, CO 80223: On and before 12/14/11, the employer did not ensure that employees required to use a tight-fitting facepiece respirator were fit tested prior to initial use of the respirator. The employer requires N95 filtering facepiece respirators while performing fabrication of granite and manufactured stone products. This condition exposes employees to a respiratory hazard due to improper fit of the respirator. Cornerstone Granite Inc. was previously cited for a violation of the same or substantially similar condition of this Occupational Safety and Health Administration Standard, 29 CFR 1910.134(f)(2), which was contained in OSHA Inspection number 314659293, Citation number 2, Item 1b issued on December 8, 2010, and became Final Order on January 3, 2011, with respect to a workplace located at 940 S. Jason St. Unit 9, Denver, CO 80223.

29 CFR 1910.134(k)(1): The employer did not provide comprehensive, understandable annual respirator training prior to employee use of respirators in the workplace: (a) Cornerstone Granite Inc. at 940 S. Jason St. Unit 9, Denver, CO 80223: On and before 12/14/11, the employer did not provide respirator training prior to issuing N95 filtering facepiece respirators. This condition exposed employees to respiratory hazards. This condition exposed the employees to a respiratory hazard. Abatement note: The employer shall ensure that each employee who uses a respirator can demonstrate knowledge of at least the following: (a) Why the respirator is necessary and how improper fit, usage, or maintenance can compromise the protective effect of the respirator; (b) What the limitations and capabilities of the respirator are; (c) How to use the respirator effectively in emergency situations, including situations in which the respirator malfunctions; (d) How to inspect, put on and remove, use, and check the seals of the respirator; (e) What the procedures are for maintenance and storage of the respirator; (f) How to recognize medical signs and symptoms that may limit or prevent the effective use of respirators; and (g) The general requirements of this section.
