29 CFR 1926.417(b): Equipment and circuits. Equipment or circuits that were deenergized were not rendered inoperative and did not have tags attached at all points where such equipment or circuits can be energized:    (a) In mechanical room 355 on or about December 14, 2011, there was no tag attached to the deenergized circuit breaker that can energize the lighting circuit in storage room 542.  The lighting circuit wire from storage room 542 was connected to this breaker, and there was power to circuit breaker panel.  Employees installed 3-way light switches in storage room 542.  The cover was locked on circuit breaker panel in mechanical room 355, and two electricians had keys to unlock the panel cover.  There was warning tape to prevent entry to the area where the circuit breaker boxes were located in mechanical room 355.  Employees stated the employer did not allow work on energized equipment.
