29 CFR 1926.451(g)(1):     Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:    On or about 12/13/11 during construction activities including but not limited to roofing work from an elevated platform, the employer did not ensure employees working from an elevated platform were protected from fall hazards approximately 17.4 feet by a guardrail system.

29 CFR 1926.501(b)(13):     Employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph (b)  Note:  If using these systems was infeasible or created a greater hazard, the employer should develop and implement a fall protection plan meeting the requirements of paragraph (k) of 1926.502:    On or about December 13, 2011, during construction activities including but not limited to roofing activities, the employer did not ensure employees were protected from fall hazards approximately 20.2 feet by ensuring a fall protection system/device was being used.

29 CFR 1926.1053(b)(1):     Where a portable ladder was used for access to an upper level landing surface and the ladders length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigied support that will not deflect, and/or a grasping device was not provided to assist emmployees in mounting and dismounting the ladder:    On or about 12/13/11 during construction activities including but not limited to roofing work, the employer did not ensure employees using a ladder to access the roof from the elevated platform was properly extended above the surface at the top with a grasping device provided.
