29 CFR 1910.147(d)(4)(i):  Lockout or tagout devices were not affixed to each energy isolating device by authorized employees:    Percoco Marble & Tile Company Inc, at 1280 W Bayaud Avenue, Denver, CO: On or about 11/14/11, Percoco Marble & Tile Company Inc did not ensure that a lockout or tagout device was affixed to each energy isolating device by authorized employees.  One employee replaced the motor pulley of the Filli Zambon Hydro AE bridge saw to enable cutting of glass rather than stone.  The employee deenergized the bridge saw prior to work.  The employer did not ensure that a lock was placed on the disconnecting means used to deenergize the bridge saw.  This condition exposed the employee to an electrical hazard.

Employees and contractors perform service upon the Breton NC350 CNC machine, Park Industries ProEdge III, and Ordway bridge polisher.  Each piece of equipment has multiple energy sources, electrical and pneumatic.  The employer's energy control procedures for each piece of equipment did not specifically outline the steps for shutting down, isolating, and securing to control energy.  This condition potentially exposed employees and contractors to hazardous energy sources.
