29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided which will prevent pull from being directly transmitted to joints or terminal screws: (a) At the project on or about December 15, 2011, the extension cord's outer insulation was not secured to the plug at a 10,000 watt portable generator. The cord supplied power to a 110 VAC Milwaukee band saw. The cord was plugged into a ground-fault circuit-interrupter outlet on the generator.
