29 CFR 1926.51(f)(1):     The employer shall provide adequate washing facilities for employees engaged in the application ofpaints, coating, herbicides, or insecticides, or in other operations where contaminants may be harmful to the employees. Such facilities shall be innear proximity to the worksite and shall be so equipped as to enable employees to remove such substances.    a.   On the east side scaffold where mortar was used which contained Portland Cement, clean water, towels and non alkaline soap was not provided where employees were able to gain access to the material in case of an emergency thereby exposing employees to a contact hazard.    b.   On the West side of the building where mortar was used which contained Portland Cement, clean water, clean towels and non alkaline soap was not provided where employees could gain access to the material in case of an emergency thereby exposing employees to a contact hazard.

29 CFR 1926.451(b)(1)(i):     Each platform unit (e.g., scaffold plank, fabricated plank, fabricated deck, or fabricated platform) shall be installed so that the space between adjacent units and the space between the platform and the uprights is no more than 1 inch (2.5cm) wide, except where the employer can demonstrate that a wider space is necessary (for example, to fit around uprights when side brackets are used to extend the width of the platform).    a.   On the 4th level of north side of the building where masonry work was performed, there was an opening in the walkway planking which measured 12 inches by 42 inches thereby exposing employees to a trip/fall hazard.

29 CFR 1926.451(e)(4)(xii):     Guardrails meeting the requirements of paragraph (g)(4) of this section shall be provided onthe open sides and ends of each landing.      a.   On the third level of the building where employees had to access a landing to gain access to the mobile scaffold, guardrails were not installed on all ends thereby exposing employees to a fall hazard.

29 CFR 1926.451(g)(4)(i):     Guardrail systems shall be installed along all open sides and ends of platforms. Guardrailsystems shall be installed before the scaffold is released for use by employees other than erection/dismantling crews.      a.   On the west side of the hydro mobile scaffold where employees were installing brick on the side of the building, there was a section of guardrail missing thereby exposing employees to a 20 foot fall fazard.
