29 CFR 1910.147(d)(4)(i):     Lockout or tagout devices shall be affixed to each energy isolating device by authorized employees.    a) Lockout device was not attached to the energy isolating point on pallet bandsaw #3 during a adjustemnt of tension of the bandsaw blade.    b) On or about December 14, 2011 a lockout device was attached by only one employee when two employees were involved in removal of a bandsaw blade.

29 CFR 1910.303(g)(2)(i):     Except as elsewhere required or permitted by this standard, live parts of electric equipment operating at 50 volts or more shall be guarded against accidentalcontact by use of approved cabinets or other forms of approved enclosures or by any of the following means:    a) Employees were exposed to shock, and/or electrocution hazard(s) when plugging in the laptop inside the open cabinet of the Kiln Controller panel, exposing them to live parts.    b) Employees were exposed to shock hazards in that the control panel cover for the trim saw line was open and could not be closed or secured exposing employees to contact.    c) In the wood stacking area of the Kilm, the motor for the fan was not covered exposing employees to live parts (terminal screw).    d) In the incline saw cutting area the juction box for the lighting system was missing a cover exposing to live parts.

29 CFR 1910.305(g)(2)(iii):     Flexible cords and cables shall be connected to devices and fittings so that strain relief is provided that will prevent pull from being directly transmitted to joints or terminal screws.    a) Flexible cord attached to a control box for the Pallet bandsaw was not equipped with strain relief to prevent movement or damage to conductors.    b) Flexible cord attached to conduit going to the junction box in the build area was not equipped with strain relief to prevent movement or damage to conductors.
