29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered. (a) At the 1700 Coral Avenue, Waukesha, Wisconsin jobsite, an employee was installing snow clips on the commercial roof without utilizing methods of fall protection. The employee was exposed to a 16-foot fall to concrete.
