29 CFR 1910.95(g)(5)(i):     The employer did not establish, within 6 months of an employee's first exposure at or above the action level, a valid baseline audiogram against which subsequent audiograms could be compared:    On or about January 10, 2012, an employee in the cleaning department was exposed to hearing loss from noise levels above the PEL while performing bench grinding operations, and the employer failed to provide a base line audiogram within six months of the employees initial exposure to the noise. On or about April 26, 2012, employees in the cleaning department were exposed to respiratory hazards from Silica and Respirable Dusts while performing castings grinding and blasting operations, and the employer failed to ensure employees did not generate large amounts of dust in their work areas in the following instances:      a. Employees were using an air gun to blow the Silica and Respirable dust off of the clothes on their bodies after removing their respirators during breaks and at the end of their work day.    b. Employees were agitating their clothes in order to remove the Silica and Respirable dust off of their clothes after removing their respirators during breaks and at the end of their work day.    Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1910.1000(c):     An employee(s) was exposed to Silica and respirable dust in excess of the 8-hour time weighted average limits listed in Table Z-3:    Employees were exposed to Silica Dust above the Permissible Exposure Limits in the following instances:    a) On or about April 26, 2012, Tumbler Blast Operator, Matias Arreguin, was exposed to 2.72 mg/m3 of Silica Dust as an 8 hour TWA (a severity of 2.1 times the Permissible Exposure Limit of 1.32 mg/m3). Sampling was performed for 451 minutes. Exposure calculations include a zero increment for the 29 minutes not sampled.    b) On or about April 26, 2012, Swing Grinder, Juan Moreno, was exposed to 2.40 mg/m3 of Silica Dust as an 8 hour TWA (a severity of 2.1 times the Permissible Exposure Limit of 1.12 mg/m3). Sampling was performed for 454 minutes. Exposure calculations include a zero increment for the 26 minutes not sampled.    c) On or about April 26, 2012, Booth Grinder, Daniel Ruteledge, was exposed to 2.44 mg/m3 of Silica Dust as an 8 hour TWA (a severity of 3.2 times the Permissible Exposure Limit of 0.77 mg/m3). Sampling was performed for 458 minutes. Exposure calculations include a zero increment for the 22 minutes not sampled.    d) On or about April 26, 2012, Bench Grinder, Miguel Sandoval, was exposed to 6.09 mg/m3 of Silica Dust as an 8 hour TWA (a severity of 11 times the Permissible Exposure Limit of 0.53 mg/m3). Sampling was performed for 465 minutes. Exposure calculations include a zero increment for the 15 minutes not sampled.    e) On or about January 10, 2012, Table Blast Operator, Jesus Garcia, was exposed to 1.60 mg/m3 of Silica Dust as an 8 hour TWA (a severity of 1.9 times the Permissible Exposure Limit of 0.833 mg/m3). Sampling was performed for 360 minutes. Exposure calculations include a zero increment for the 120 minutes not sampled.    Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.1000(e):     Feasible administrative or engineering controls were not determined and implemented to achieve compliance with the limits prescribed in 29 CFR 1910.1000(a) through (d):    On or about April 26, 2012, employees performing grinding and blasting operations were exposed to airborne concentrations of Silica and Respirable Dusts that exceeded their permissible exposure limits.     General methods of control applicable in these circumstances include, but are not limited to the following:    1) The use of Local Exhaust Ventilation (LEV) during grinding and blasting operations    2) Using materials that dismiss lower concentrations of Silica and Respirable Dusts.    Abatement Schedule:    Step 1:  Submit to the Area Director a written, detailed plan of abatement outlining a schedule for the implementation of engineering and/or administrative measures to control employee exposure to hazardous substances as referenced in this citation.  This plan shall include, at a minimum, target dates for the following actions which must be consistent with the abatement dates required by this citation:    1.	Evaluation of engineering/administrative control options;  2.	Selection of optimum control methods and completion of design;  3.	Procurement, installation and operation of selected control measures;  4.	Testing and acceptance or modification/redesign of controls.    All proposed control measures shall be approved for each particular use by a competent industrial hygienist or other technically qualified person.  90-day progress reports are required during the abatement period.    Step 3:  Abatement shall have been completed by the implementation of feasible engineering controls and/or administrative controls upon verification of their effectiveness in achieving compliance.
