29 CFR 1915.71(k)(2): Ramps and stairways was not provided with 36-inch handrails with midrails. A 6-foot metal ramp that was not equipped with 36 inch handrails with midrails, was used by employees to cross over to a different section of the dry dock. The ramp was 15 feet above the water exposing employees to a fall hazard.

29 CFR 1915.72(a)(1): The use of ladders with broken or missing rungs or steps, broken or split side rails, or otherfaulty or defective construction is prohibited. When ladders with such defects are discovered, they shall be immediately withdrawn from service. Inspection of metal ladders shall include checking for corrosion of interiors of open end, hollow rungs. A damaged 12 foot metal ladder was used by employees to access the top of a dry dock. The metal ladder was bent on the right side.

1915.93(c)(1): The employer did not ensure that each motor vehicle is maintained in a serviceable and safe operating condition, and removed from service if it is not in such condition. The Hyster Powered industrial truck with defaced model and S.N., was not withdrawn from service until restored to safe operating condition. The forklift did not have a working horn to warn employees working close by.
