29 CFR 1926.501(b)(3): Each employee in a hoist area was not protected from falling 6 feet (1.8m) or more to lower levels by guardrail systems or personal fall arrest systems: a) Employees were exposed to the hazard of falling approximately 9 feet from the edge of the roof deck while signaling a crane and receiving delivery of materials being delivered to the roof deck. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system: a) East edge of roof deck - Employees were exposed to the hazard of falling approximately 25 feet from the edge of the roof deck while engaged in roofing activities outside of warning lines where fall protection or safety monitoring was not provided. a) South edge of roof deck - Employees were exposed to the hazard of falling approximately 9 feet from the roof edge while engaged in roofing activities without the use of fall protection or a safety monitoring system. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records. Nations Roof North, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501 (b)(10) which was contained in OSHA inspection number 311246839, citation 1, item 1, issued on November 16, 2007 and became final order on or about December 11, 2007.
