29 CFR 1910.23(c)(1): Open-sided floor(s) or platform(s) 4 feet or more above the adjacent floor or ground level were not guarded by standard railings (or the equivalent as specified in 29 CFR 1910.23(e)(3)(i) through (v)), on all open sides: a) Warehouse Freezer- On or about 12/15/11, employees were required to work from a pallet elevated by a forklift 30 feet in the air without a standard railing system. Employees were exposed to the hazards associated with falls.

29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator was competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l) a) Park Packing Chicago- On or about 12/15/11, employees were required to operate forklifts without training and evaluation from the employer to ensure competency in the use of the equipment.
