29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket): On or about January 19, 2012, employees in Kiln Area were exposed to electrical shock from a damaged flexible cord on a floor fan and the employer failed to effectively implement inspections of the electrical equipment. Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1910.134(c)(2)(ii): The employer did not establish and implement those elements of a written program necessary to ensure that any employee using a respirator voluntarily was medically able to use that respirator, and that the respirator was cleaned, stored, and maintained so that its use does not present a health hazard to the user: On or about January 19, 2012, an employee was allowed to use a tight fitting respirator with cartridges and the employer failed to develop and implement a written respiratory protection program that included employee medical evaluations and training. Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).
