29 CFR 1926.454(a): The employer shall have each employee who performs work while on a scaffold trained by a personqualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to controlor minimize those hazards. The training shall include the following areas, as applicable:    a.  On or about December 19, 2011 - at the above addressed jobsite, an employee conducting roofing operations from a ladder jack scaffold, approximatley twenty one (21) feet three (3) inches above the ground was not trained to recognize the hazards associated with scaffolding.  The employee was thereby exposed to the hazards associated with falls.
