29 CFR 1915.158(b)(3): At least one 30-inch (0.76 m) U. S. Coast Guard approved ring life buoy with a line attached was not located on each staging alongside of a floating vessel on which work is being performed. a) Keehi Marine Center, haul out slip: Two yard workers were exposed to a fall into eight feet deep of water while jumping twenty four (24) inches from dock to vessel and there was no 30-inch U.S. Coast Guard approved lifering located near the vessel.

29 CFR 1915.159(d): Before using personal fall arrest equipment, each affected employee was not trained to understand The application limits of the equipment and proper hook-up, anchoring, and tie-off techniques. a) Keehi Marine Center: A yard worker operating an aerial lift was not trained to understand the proper use of the fall protection system he was using. He was wearing a harness with a lanyard which would allow him to fall seven and one half feet while exposed to a fall of six feet.
