Workers using respirators were not given a physical exam and a respirator fit test.

Acute and chronic reactions to these hazardous materials can also cause skin diseases and dermatitis.

The establishment must be recorded on the form.     b) For 2008, 2009 and 2010 Log OSHA Form 300A: The average number of employee hours did not reflect the actual number of employee hours worked for each of the three years.      c) For 2008, 2009 and 2010 Log OSHA Form 300A: The totals were missing, including zeros indicating none, at the bottom of the columns for all years reviewed.       d) For 2010 Log OSHA Form 300A had incorrect entries for "H - Total number of cases with days away from work" and "K - Total number of days away from work"
