29 CFR 1910.134(c)(1):     The employer did not ensure that a written respiratory protection program with worksite-specific procedures was updated as necessary to reflect those changes in workplace conditions that affect respirator use, to include the provisions of (i)-(ix), as applicable: This Standard is applicable to ship repair operations by 29 CFR 1915.154 that incorporates the General Industry requirement of 1910.134.    a) Keehi Marine Center, boatyard:	Workers are allowed to use half mask respirators on a variety of jobs but not limited to painting, corrosion control and treatment work, surface (hull) sanding and sandblasting, fiberglass repairs among others. Workers using respirators were not given a physical exam and a respirator fit test.

29 CFR 1910.1200(e)(1):     The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met:  a) Keehi Marine Center, boatyard: On or about 14 NOV 11 and at times prior thereto, a written hazard communication program was not developed, implemented and maintained for hazardous materials such as, but not limited to Non slip Deck Coating- MS 4006, Interspeed 640 Antifouling materials, acetone and Denature Alcohol for cleaning and wiping.  Acute and chronic reactions to these hazardous materials can also cause skin diseases and dermatitis.

29 CFR 1904.29(b)(1):  The Log of all Work-Related Injures and Illnesses (OSHA Form 300) and/or the Summary of Work-Related Injuries and Illnesses (OSHA Form 300-A), and/or equivalent forms were not recorded in detail required by the respective forms.     1. Keehi Marine Center: On or about 14 NOV 11, the OSHA 300 logs and 300A were not filled out in accordance with the regulation:     a) For 2008 and 2009 Log OSHA Form 300: On the top right of the form, the name of the establishment was missing. The establishment must be recorded on the form.     b) For 2008, 2009 and 2010 Log OSHA Form 300A: The average number of employee hours did not reflect the actual number of employee hours worked for each of the three years.      c) For 2008, 2009 and 2010 Log OSHA Form 300A: The totals were missing, including zeros indicating none, at the bottom of the columns for all years reviewed.       d) For 2010 Log OSHA Form 300A had incorrect entries for "H - Total number of cases with days away from work" and "K - Total number of days away from work"
