29 CFR 1926.501(b)(1):   Each employee on a walking/working surface with an unprotected side or edge which is 6 feet or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:    a)  Employees were exposed to the hazard of falling while dismantling a metal fire escape and were exposed to a fall hazard of up to approximately 27 feet.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase  or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1926.1051(a):  Stairways or ladders were not provided  at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, and no ramp, runway, sloped embankment, or personnel hoist was provided:    a) Employees accessed the work platform erected over the sidewalk by using an extension ladder and climbing over the plywood wall erected around the edge of the platform. The top of the plywood wall to the deck of the work platform was approximately 48 inches and no means of safe access/egress to/from the work platform was provided.    In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
