29 CFR 1910.212(a)(1): Machine guarding was not provided to protect operator(s) and other employees from hazard(s) created by pinch points: a. Specials Department - On or about December 12, 2012, employees were required to operate the Chicago Dries and & Krump press brake. The unitized dies and the rear of the press brake were unguarded. b. Specials Department - On or about December 12, 2012, employees were required to operate the Enco hydraulic shear. The holdown clamp was unguarded. The employees were thereby exposed to the hazards associated with pinch points.

Entire facility - On or about April 9, 2010, the employer's periodic inspections of established energy control procedures for 2008, 2009 and 2011 were not conducted as required. Employees who perform servicing and maintenance on such equipment including, but not limited to, the pleaters and nova lines were thereby exposed to the hazards associated with the unexpected energization of equipment.
