Specials Department - On or about December 22, 2012, employees were required to operate a Chicago Dries & Krump press brake with an unguarded foot treadle.  The employees were thereby exposed to struck by-hazards.        ***This item requires documentation of abatement***

29 CFR 1910.212(a)(1):     Machine guarding was not provided to protect operator(s) and other employees from hazard(s) created by pinch points:     a. Specials Department - On or about December 12, 2012, employees were required to operate the Chicago Dries and & Krump press brake.  The unitized dies and the rear of the press brake were unguarded.     b. Specials Department - On or about December 12, 2012, employees were required to operate the Enco hydraulic shear. The holdown clamp was unguarded.    The employees were thereby exposed to the hazards associated with pinch points.

29 CFR 1910.147(c)(6)(i):     The employer shall conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirements of this standard are being followed.    a. Entire facility - On or about April 9, 2010, the employer's periodic inspections of established energy control procedures for 2008, 2009 and 2011 were not conducted as required.  Employees who perform servicing and maintenance on such equipment including, but not limited to, the pleaters and nova lines were thereby exposed to the hazards associated with the unexpected energization of equipment.

The employer shall maintain a written copy of the procedures outlined in paragraph(b)(2) and shall make it available for inspection by employees and by the Assistant Secretary of Labor and his or her authorized representatives. Note: The written procedures may be in the form of a copy of paragraph (b) of this section.    a. Facility - On or about December 22, 2012, the employer did not maintain written electrical safety-related work practice procedures outlined in 29 CFR 1910.333(b)(2) for employee(s) who were required to perform new equipment installation (Nova Line #1) using a 480 Volt motor control center.  The employee(s) was thereby exposed to hazards of electrical shock and arc flash/blasts.

29 CFR 1910.303(b)(2):     'Installation and use. Listed or labeled equipment shall be installed and used in accordance with any instructions included in the listing or labeling.      a. Nova Lines - On or about December 22, 2012, Type TC tray cables used to power the decoiler turntables did not meet Code requirements for use as exposed wiring.  The tray cables were not used in accordance with the listing or labeling instructions.
