29 CFR 1926.51(f)(1): The employer shall provide adequate washing facilities for employees engaged in the application ofpaints, coating, herbicides, or insecticides, or in other operations where contaminants may be harmful to the employees. Such facilities shall be innear proximity to the worksite and shall be so equipped as to enable employees to remove such substances. a. On the site where employees used mortar which contained chemicals such as Portland Cement, the employer did not provide clean water, clean towels and non alkaline soap thereby exposing employees to a contact hazard.

29 CFR 1926.451(g)(4)(i): Guardrail systems shall be installed along all open sides and ends of platforms. Guardrailsystems shall be installed before the scaffold is released for use by employees other than erection/dismantling crews. a. On the southwest corner of the front scaffold, there was a 20 inch x 6 feet opening in the front of the scaffold thereby exposing employees to a fall hazard.

29 CFR 1926.451(h)(1): In addition to wearing hardhats each employee on a scaffold shall be provided with additionalprotection from falling hand tools, debris, and other small objects through the installation of toeboards, screens, or guardrail systems, or throughthe erection of debris nets, catch platforms, or canopy structures that contain or deflect the falling objects. When the falling objects are toolarge, heavy or massive to be contained or deflected by any of the above-listed measures, the employer shall place such potential falling objects awayfrom the edge of the surface from which they could fall and shall secure those materials as necessary to prevent their falling. a. On the tubular welded frame scaffold where employees were laying block, the employer did not use another meathod of over head protection to supply overhead protection for employees working below the scaffold thereby exposing employees to a struck-by hazard.

29 CFR 1926.452(c)(5)(iii): Be used only to support personnel, unless the scaffold has been designed for other loads by aqualified engineer and built to withstand the tipping forces caused by those other loads being placed on the bracket-supported section of thescaffold. a. On the west side of the north classrooms, the employer piled bricks, block and tools on the outrigger away from the building which should have been used for personnel only thereby exposing employees to a struck-by hazard.
