29 CFR 1926.502(f)(1)(i): When mechanical equipment is not being used, the warning line shall be erected not less than 6 feet (1.8 m) from the roof edge. a. On or about December 22, 2011, at the above addressed jobsite, southwest side of the roof, warning lines were erected closer than six (6) feet from the roof edge. Employees were thereby exposed to the hazards associated with falls of twenty-four (24) feet falls.

29 CFR 1926.502(h)(1)(v): The safety monitor shall not have other responsibilities which could take the monitor's attention from the monitoring function. a. On or about December 22, 2011, at the above addressed jobsite, employees conducting roofing operations were exposed to fall hazards of twenty-four (24) feet, due to the safety monitor having other responsibilities, such as but not limited to, roofing operations. THE J.L. ADLWER ROOFING AND SHEET METAL, INC., WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 29 CFR 1926.502(h)(1)(V), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 311250237, CITATION NUMBER 2, ITEM NUMBER 1 AND WAS AFFIRMED AS A FINAL ORDER ON JUNE 23, 2008, WITH RESPECT TO A WORKPLACE LOCATED AT 105 EAST 4TH AVE, NAPERVILLE, IL 60540.
