Paragraphs (g)(1)(i) through (vii) of this section establish the types of fall protection to be provided to the employees on each type of scaffold.    a.  On or about December 19, 2011, at the above addressed jobsite, an employee was performing power washing operations from a home made box attached to a Caterpillar rough terrain forklift model #TH63.  The box did not have guardrails installed, thereby exposing the employee to a fall of twenty five (25) feet.
